administrator. We have a dedicated representative that you can email at
susan.benefits@P5health.com with questions. Open
enrollment is every April and any changes to your insurance at that time will be
effective May 1st. The cost is $125 for the first dependent, $50 for each
additional dependent, and a cap of $275 (in other words, the maximum anyone
would pay is $275, even if there are more than 4 dependents.)

The Employee
Referral Program provides compensation to employees for referring qualified
applicants hired with the company. After the referred employee has been employed
for 30 days, the employee referring him/her will be entitled to $150. Upon
completion of 90 days, the second $150 will be paid. On the first day of
employment by the new employee, the referring employee must submit two manual
check requests for $150 each dated 30 and 90 days from the hire date. Both
requests must be signed by the new employee's Property Supervisor and forwarded
to Corporate Accounting.
